Before continuing note the following: MRT transponders DO NOT need
to be sent if you are ordering mPTX matched ID number transponders.
Original transponders are only sent for clone or clone trade in orders.
If matching your existing MRT ID number skip clone form, instead refer
to the Knowledge Base article MRT mPTX Matched ID Ordering
If you have an old style red 'DP' transponder to be cloned send it along
with the clone form (PDF attached below or at http://www.rclaps.com )
Details are on the form, fill in whatever you can, enter 'Order Reference'
if known (for fastest order processing and turn around time - email MRT
support to request your personal order ref / Offer Code for discount).

For Trade In orders YES / NO must be marked as yes, (note original RC
transponder sent will NOT be returned). Broken wires or damaged cases
are not a problem, the transponder will still qualify for the 'Trade In Offer'.
Not knowing your transponder ID number is ok, leave 'ID number' blank.
